Why can’t I get a 90-day script on all of my medications?

While a 90-day script may be appropriate for conditions that do not vary over time they are not appropriate for all medications. New prescriptions are not given for 90-day supplies due to the need to evaluate and adjust the dosing until the desired therapeutic results are achieved. Many medications require monitoring more than once per year so a yearlong script will not be provided. Adjusting dosing and medications monitoring outweigh the demands of the insurance companies when it comes to patient care.

Why do I need to be seen in the office

It is impossible to help you achieve optimum health if face to face appointments are not done at regular intervals. Patient must be seen at least once per year to be maintained as an active patient in the office. No medications will be refilled beyond a year from the previous appointment. Many of the conditions treated in the office require monitoring at 3 to 6 month intervals and if a patient is not compliant with the monitoring, then treatment cannot be continued.

Why did my medication refills get denied?

There are many laws governing how medications are prescribed and recommended monitoring. Any controlled substance prescription will require in-person office visits and laboratory tests as appropriate based on which medication has been prescribed and what is being treated. All other scripts require office visits and labs based on what is being treated and the monitoring requirements/recommendations

Can I do a telehealth appointment?

This is an ever changing topic. The federal government is constantly changing the regulations concerning the availability of telehealth. At present, the office does offer telehealth options, but not all appointments qualify. Controlled substances cannot be prescribed via telehealth. An excellent use of telehealth is reviewing labs and answering medication questions. Patients must reside in Utah to do a telehealth appointment. The Doctor is only licensed in Utah and will only see out-of-state patients in person in the office. This is necessary to avoid fines for practicing across state lines.

How often do I need to be seen for an appointment?

Depends on what issues you are being treated for. (list is not inclusive of all diagnoses or appointment types)

Thyroid – if stable on dosing, an appointment every 6 months. Lab work required every 6 months.

Thyroid – if unstable and dosing is constantly changing or patient has Hashimoto’s Thyroiditis – appointment needed with labs every 3 months.

ADHD – appointments are required every month until dosing is stable, then every 3 months. Controlled substance medication laws require in person appointments at least every 3 months. Labs are required at least once per year.

Pain Management – If stable on current dosing, patient will need an appointment every 3 months, random drug testing may be required, and labs are required every 6 months.

Anxiety medications including benzodiazepines – if stable on current dosing without medication misuse, appointment with labs is required every 6 months.

Psychiatric Medications – if stable on current dosing, an appointment is needed every 6 months with labs.

Cardiovascular / Hypertension – depends on medication and whether the patient is stable on the current dosing. An appointment and labs are done at 3 to 6 month intervals. Lipids testing is every 6 – 12 months.

Diabetes – appointment is needed every 3 to 6 months with labs if stable on current dose. If dosing is not stable, then appointments will be needed every 3 months with labs.

How do I get a lab order before I have my appointment?

You don’t. Lab orders are based on the current problems and new problems discussed in the appointment, it is not possible to provide the patient with an advance order. I have too many patients that have gotten angry because I did not order the labs that they thought they needed. I cannot read the minds of my patients, please make an appointment before you need a lab order.

Why do you require labs?

Labs provide the information necessary to support initiating or changing a treatment. There are different labs for different purposes, and the doctor will consult you about the labs prior to ordering them but the doctor will decide which labs are required to evaluate your condition.

Why do I have a bill for the office visit? I have insurance.

The doctor does not know the details of your medical plan and it is your responsibility to know what is covered. If you have a high-deductible policy, most of the insurance companies will not pay anything on office visits until you meet the out-of-pocket deductible per individual or family. If we are a provider for your insurance, the bill you receive from the office visit is the discounted bill at a rate dictated by your insurance provider. If we are not a provider for your insurer or you have a health share plan, we bill you the discounted office price, cash pay patients receive the same discount.

The DMV is requiring me to get the Functional Ability Form completed. Can you just fill it out for me and send it in?

No. You need an appointment and depending on how long it has been since you had an appointment and what is being treated, you may also need lab work. The DMV form is a legal document, and the Doctor is required to be able to demonstrate adequate evaluation and monitoring for the condition being attested to on the form. The state now requires the date of the last in-person appointment and they may not accept an appointment older than 2 months. Patient must sign the form, and then it is faxed from the office to the DMV

 

Any patient request that, in the opinion of the Doctor, has the potential to adversely affect patient care, threaten the Doctor’s licensing, DEA, Malpractice Insurance, or livelihood, will be denied.
